前路减压纳米羟基磷灰石/聚酰胺66支撑体植骨融合术治疗脊髓型颈椎病的中期疗效_《中国修复重建外科杂志》

目的评价前路减压、纳米羟基磷灰石/聚酰胺66（nano-hydroxyapatite/polyamide 66，n-HA/PA66）支撑体植骨融合术治疗脊髓型颈椎病的中期疗效。方法回顾性分析2008年8月－2010年1月行前路减压、n-HA/PA66支撑体植骨融合术治疗的48例脊髓型颈椎病患者临床资料。男33例，女15例；年龄42～72岁，平均54.5岁。病程3～12个月，平均6个月。病变节段：单节段35例，其中C3、4 7例，C4、5 18例，C5、6 10例；双节段13例，其中C3～5 7 例，C4～6 6例。椎间盘突出症28例，后纵韧带骨化症12例，椎体后缘骨赘致压8例。行单节段椎体开槽减压植骨融合术35例，单节段椎间盘切除减压植骨融合术13例。手术前后摄颈椎正侧位X线片，测量融合节段高度并计算支撑体下沉距离，测量融合节段前凸角；行CT三维重建，采用Brantigan等的5级评定标准评估植骨融合情况；采用疼痛视觉模拟评分（VAS）及日本骨科协会（JOA）颈椎评分法评估患者症状改善情况。结果术后48例均获随访，随访时间36～54个月，平均46个月。均未出现支撑体破裂、移位或脱出等情况。末次随访时按Brantigan等的分级标准评价植骨融合：获E级36例，D级10例，C级2例，植骨融合率为96%。术后即刻、6个月及末次随访时融合节段高度及前凸角均较术前显著提高（P lt; 0.05），术后各时间点间差异均无统计学意义（P gt; 0.05）。末次随访时支撑体下沉距离为（1.3 ± 1.0） mm。术后6个月及末次随访时患者VAS及JOA评分均明显优于术前，末次随访时优于术后6个月，比较差异均有统计学意义（P lt; 0.05）。结论n-HA/PA66 支撑体能有效恢复及维持融合节段生理高度及弧度、显著促进植骨融合，便于术后手术节段的影像学观察，应用该支撑体行前路减压植骨融合术治疗脊髓型颈椎病的中期疗效满意。
