目的 　比較股骨近端抗旋髓內(nèi)釘（PFNA）與動(dòng)力髖螺釘（DHS）兩種方法內(nèi)固定治療高齡股骨粗隆間骨折的療效。 方法　 2007年1月－2011年1月分別采用PFNA、DHS治療高齡股骨間粗隆骨折（EvansⅠ～Ⅳ型）共68例，其中PFNA組36例，平均年齡81.5歲；DHS組32例，平均年齡82.3歲。兩組患者在年齡、性別、骨折分型、合并癥以及受傷至手術(shù)時(shí)間方面比較差異均無(wú)統(tǒng)計(jì)學(xué)意義（P＞0.05），有可比性。 結(jié)果　 術(shù)后68例患者均獲隨訪，隨訪時(shí)間6～26個(gè)月。與DHS組比較，PFNA組手術(shù)時(shí)間短、術(shù)中出血少，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）；PFNA組骨性愈合時(shí)間短，但兩組差異無(wú)統(tǒng)計(jì)學(xué)意義（P＞0.05）。術(shù)后DHS組出現(xiàn)1例髖內(nèi)翻、1例拉力螺釘切出股骨頭頸，PFNA組出現(xiàn)1例主釘退釘，兩組術(shù)后并發(fā)癥發(fā)生率比較差異有統(tǒng)計(jì)學(xué)意義（P＜0.01）。術(shù)后1年P(guān)FNA組髖關(guān)節(jié)Harris評(píng)分顯著高于DHS組，差異有統(tǒng)計(jì)學(xué)意義（P＜0.01）。 結(jié)論 　PFNA與DHS兩種固定方法的骨性愈合時(shí)間短、術(shù)后并發(fā)癥少、髖關(guān)節(jié)功能恢復(fù)好，是治療股骨粗隆間骨折的良好方式，尤其是PFNA能減少手術(shù)時(shí)間和術(shù)中出血量，手術(shù)創(chuàng)傷小，對(duì)高齡且不能耐受較大手術(shù)者可作為一種首選術(shù)式。
