【摘要】 目的 探討抗旋股骨近端髓內(nèi)釘（proximal femoral nail anti-rotation，PFNA）治療老年股骨粗隆間骨折的臨床療效。 方法 2007年1月-2010年1月，對(duì)36例老年股骨粗隆間骨折行閉合復(fù)位PFNA內(nèi)固定。其中男21例，女15例；年齡60～92歲，平均68歲。骨折按Evans分型:Ⅰ型3例，Ⅱ型8例，Ⅲ型15例，Ⅳ型10例。均為閉合骨折。受傷至手術(shù)時(shí)間3 ~7 d。 結(jié)果 手術(shù)時(shí)間40～75 min，平均50 min。術(shù)后切口均Ⅰ期愈合。36例獲隨訪6~18個(gè)月，平均10個(gè)月。X線片示均達(dá)臨床骨折愈合，愈合時(shí)間8~22周，平均12周。無(wú)感染、脂肪栓塞、深靜脈血栓形成發(fā)生，無(wú)內(nèi)固定失敗、髖內(nèi)翻及短縮外旋畸形等并發(fā)癥發(fā)生。髖關(guān)節(jié)功能按Harris評(píng)分標(biāo)準(zhǔn)評(píng)定：優(yōu)28例，良6例，中2例，優(yōu)良率88.9%。 結(jié)論 PFNA治療老年股骨粗隆間骨折具有療效確切、手術(shù)時(shí)間短、操作步驟簡(jiǎn)便、損傷小、失血少、骨折固定確實(shí)、并發(fā)癥少、可早期下床功能鍛煉等優(yōu)點(diǎn)，是一種比較理想的髓內(nèi)固定系統(tǒng)，尤其適合老年患者。
