目的:通過(guò)檢測(cè)髓母細(xì)胞瘤（Medulloblastoma）中凋亡抑制蛋白（survivin）的表達(dá)情況，探討它的臨床意義，為髓母細(xì)胞瘤的治療尋求新的思路。方法:對(duì)50例髓母細(xì)胞瘤及10例正常腦組織石蠟切片行免疫組化檢測(cè)，通過(guò)對(duì)survivin蛋白在髓母細(xì)胞瘤患者與正常腦組織中的表達(dá)差異，并運(yùn)用相關(guān)統(tǒng)計(jì)分析方法分析它在不同性別、年齡段、腫瘤大小、病理類型、臨床分期的表達(dá)差異及其與預(yù)后的相關(guān)性。結(jié)果:（1）髓母細(xì)胞瘤中survivin蛋白陽(yáng)性表達(dá)率為72%，明顯高于正常腦組織的陽(yáng)性表達(dá)率0%，二者表達(dá)差異明顯（P lt;0.01）；亞組分析發(fā)現(xiàn)在survivin蛋白陽(yáng)性組（n=36）和陰性組（n=14）病例中，性別分布、腫瘤部位、腫瘤大小、病理分型等因素在兩者間無(wú)明顯差別（P gt;0.05），而年齡及TM臨床分期在兩組間差異顯著（P分別為0.044和0.027）。（2）survivin陽(yáng)性患者預(yù)后與和陰性組相比差異顯著（P=0.001）。結(jié)論:survivin可能參與了對(duì)髓母細(xì)胞瘤細(xì)胞凋亡的調(diào)控，并對(duì)髓母細(xì)胞瘤的發(fā)生發(fā)展可能起著重要作用。由于survivin蛋白表達(dá)與髓母細(xì)胞瘤患者年齡、臨床分期及預(yù)后等也密切相關(guān)，可作為判斷病情和評(píng)價(jià)預(yù)后的指標(biāo)。
