目的 　探討并總結(jié)急診斷流術(shù)對(duì)門靜脈高壓癥并發(fā)上消化道大出血的治療效果與經(jīng)驗(yàn)。
方法 　回顧性分析2006年3月至2011年7月期間中國醫(yī)科大學(xué)附屬盛京醫(yī)院急診行斷流術(shù)治療的42例門靜脈高壓癥并發(fā)上消化道大出血患者的臨床資料。
結(jié)果 　29例急診行脾切除＋賁門周圍血管離斷術(shù)，8例急診行脾動(dòng)脈結(jié)扎＋賁門周圍血管離斷術(shù)，5例急診行單純賁門周圍血管離斷術(shù)。術(shù)后3h內(nèi)手術(shù)止血率為100%，1例于術(shù)后第8天因肝功能衰竭死亡，3例術(shù)后2d內(nèi)出現(xiàn)腹腔滲血，經(jīng)保守治療后滲血停止。其余患者術(shù)后經(jīng)2～4周治療后均順利出院。術(shù)后1年常規(guī)隨訪，5例失訪，36例獲訪患者中1例于術(shù)后8個(gè)月再次出血，經(jīng)胃鏡套扎成功止血； 1例于術(shù)后7個(gè)月體檢發(fā)現(xiàn)原發(fā)性肝癌，行肝部分切除術(shù)；其余患者可以進(jìn)行一般的日常工作和生活。
結(jié)論 　對(duì)于門靜脈高壓癥并發(fā)上消化道大出血者，把握恰當(dāng)?shù)氖中g(shù)時(shí)機(jī)，果斷實(shí)施急診斷流術(shù)，可以迅速而有效止血，挽救患者的生命。
