目的 　總結(jié)和分析殘胃癌的治療方式及預(yù)后因素。
方法 　收集2000年3月至2008年5月期間，川北醫(yī)學(xué)院附屬第二醫(yī)院和中國人民解放軍總醫(yī)院收治的共114例殘胃癌患者的臨床病理學(xué)資料，分析其治療方式及影響預(yù)后的因素。結(jié)果　全組病例手術(shù)切除率和R0切除率分別為57.0%（65/114）及54.4%（62/114），行全胃切除、遠(yuǎn)端胃部分切除、近端胃部分切除、內(nèi)鏡下黏膜切除術(shù)（EMR）及內(nèi)鏡下黏膜剝離術(shù)（ESD）者分別占手術(shù)切除者的73.8%（48/65）、16.9%（11/65）、3.1%（2/65）、4.6%（3/65）及1.5%（1/65），分別占R0切除者的75.8%（47/62）、16.1%（10/62）、3.2%（2/62）、4.8%（3/62）及0。75例獲隨訪，隨訪時間為0.3～79.0個月，中位隨訪時間為12個月；總體中位生存時間為19.5個月，1、3及5年累積生存率分別為61.8%、42.3%和30.1%。多因素分析結(jié)果顯示，首次行遠(yuǎn)端胃切除（P=0.002）、kps評分≥80分（P=0.016），大體分型低（P=0.013）和臨床無遠(yuǎn)處轉(zhuǎn)移（P=0.000）者，手術(shù)切除率均較高； R0切除（P=0.000）、大體分型（P=0.005）和臨床T分期（P=0.006）為殘胃癌的獨立預(yù)后因素。無論是單因素還是多因素分析，原發(fā)疾病的良惡性質(zhì)均不影響預(yù)后（P＞0.05）。姑息性手術(shù)切除、姑息化療、單純剖腹手術(shù)及最佳支持治療間的總體生存曲線比較差異無統(tǒng)計學(xué)意義（P＞0.05）。
結(jié)論　 原發(fā)疾病的性質(zhì)不影響殘胃癌的治療及預(yù)后，R0切除是殘胃癌最重要的獨立預(yù)后因素； 殘胃癌的最佳手術(shù)方式是切除全部殘胃，應(yīng)盡量避免姑息性剖腹手術(shù)。
