目的  評估腹腔鏡輔助胃癌根治術(shù)后患者肝功能變化并分析其有關(guān)影響因素。
方法  收集2008年6月至2010年6月期間在解放軍總醫(yī)院行胃癌根治術(shù)及結(jié)腸癌根治術(shù)病例的臨床資料,分為開腹胃癌根治術(shù)組(OG組, n=43)、腹腔鏡輔助胃癌根治術(shù)組(LAG組, n=35)以及腹腔鏡輔助結(jié)腸癌根治術(shù)組(LAC組, n=23),比較各組患者術(shù)前及術(shù)后第1、3、5和7天血清AST、ALT、TB和ALP水平變化并分析其相關(guān)的影響因素。
結(jié)果  LAG組和OG組患者術(shù)后5 d內(nèi)血清AST和ALT水平均較術(shù)前明顯增高(P<0.05),LAC組患者術(shù)后未觀察到肝功能的變化(P>0.05); 除術(shù)后第1天LAG組血清AST和ALT水平顯著高于OG組(P=0.035和P=0.041)和術(shù)后第3天LAG組血清ALT水平顯著低于OG組(P=0.048)外,2組在其余時間點血清AST和ALT水平的差異無統(tǒng)計學(xué)意義(P>0.05); LAG組患者術(shù)后5 d內(nèi)血清AST和ALT水平均較LAC組為高(P<0.05)。LAG組和OG組患者術(shù)前及術(shù)后血清TB和ALP水平無明顯變化(P>0.05)。胃癌患者術(shù)后血清ALT水平升高與體重指數(shù)(P=0.038)、手術(shù)時間(P=0.011)、術(shù)中肝損傷(P=0.035)以及入肝血流的異常阻斷(P=0.048)有關(guān),而與手術(shù)方式無關(guān)(OG比LAG,P>0.05)。
結(jié)論  胃癌根治術(shù)后患者均有一過性肝功能損害,而造成腹腔鏡輔助胃癌根治術(shù)后患者肝功能異常的原因可能與術(shù)中對肝臟的牽拉和損傷以及對入肝血流的異常阻斷有關(guān),而非CO2氣腹的影響。對于無嚴(yán)重肝功能損害或其他重要臟器疾患的患者來說,腹腔鏡輔助胃癌根治術(shù)是安全、可行的。
