目的探討腔鏡在下肢慢性靜脈功能不全(CVI)并發(fā)靜脈性潰瘍中的臨床治療經(jīng)驗(yàn)與療效。方法回顧性分析2004年5月至2011年4月期間我院應(yīng)用腔鏡治療78例(88條患肢)下肢CVI并發(fā)靜脈性潰瘍患者的臨床資料，患者均行大隱靜脈高位結(jié)扎+腔內(nèi)激光治療(EVLT)+腔鏡深筋膜下交通靜脈離斷術(shù)(SEPS)。結(jié)果所有患者手術(shù)順利。SEPS手術(shù)時(shí)間15～30 min，平均20 min； 術(shù)中出血量1～5 ml，平均2 ml； 術(shù)后住院時(shí)間2～8 d，平均5 d。術(shù)后肢體酸脹感和曲張淺靜脈消失，色素沉著區(qū)縮小。術(shù)后筋膜下血腫3例，皮下氣腫2例，小腿脛前區(qū)及足靴區(qū)麻木感3例。所有患者4～6周潰瘍愈合，隨訪0.5～5年，平均3.5年，僅1例復(fù)發(fā)，是由于足靴區(qū)交通靜脈殘留。結(jié)論SEPS是治療CVI并發(fā)靜脈性潰瘍的首選方法，具有創(chuàng)傷小、出血少、手術(shù)時(shí)間短、恢復(fù)快、并發(fā)癥少、療效顯著等特點(diǎn)。
