目的總結(jié)腹壁疝補(bǔ)片修補(bǔ)術(shù)后感染的外科處理方法及經(jīng)驗(yàn)。方法回顧性分析我科2007年6月至2010年5月期間16例腹壁疝補(bǔ)片修補(bǔ)術(shù)后感染并接受外科處理的患者的臨床資料,其中男10例,女6例; 年齡24~73歲,平均45.2歲。其中腹壁切口疝補(bǔ)片修補(bǔ)術(shù)后感染11例,腹壁腫瘤切除術(shù)后腹壁缺損補(bǔ)片修補(bǔ)術(shù)后感染4例,回腸代膀胱造口旁疝補(bǔ)片修補(bǔ)術(shù)后感染并尿瘺1例?;颊弑憩F(xiàn)有補(bǔ)片暴露、慢性流膿、腹壁慢性竇道及腸皮瘺,均就診于初次手術(shù)的醫(yī)生,經(jīng)局部換藥處理后3~24個(gè)月未愈?;颊咴谖铱平邮芰烁涡愿腥揪W(wǎng)片切除及腹壁重建術(shù)。結(jié)果所有患者均將感染補(bǔ)片取出,5例采用成分分離技術(shù)自體組織游離修補(bǔ),4例同時(shí)應(yīng)用聚丙烯平片加強(qiáng)修補(bǔ),5例同時(shí)行脫細(xì)胞基質(zhì)生物補(bǔ)片修補(bǔ),1例未行修補(bǔ)給予切口創(chuàng)面負(fù)壓吸引加局部換藥,1例去除補(bǔ)片后未行加強(qiáng)修補(bǔ)直接縫合關(guān)閉切口。術(shù)后住院時(shí)間9~25 d,平均14 d。術(shù)后切口一期愈合13例,其余3例切口經(jīng)局部換藥二期愈合。隨訪6~34個(gè)月,平均22個(gè)月,無疝復(fù)發(fā)。結(jié)論腹壁疝或缺損補(bǔ)片修補(bǔ)術(shù)后感染的外科處理非常棘手,需根據(jù)患者個(gè)體具體情況處理方可取得滿意效果。
