• 昆明市第一人民醫(yī)院暨昆明醫(yī)學(xué)院附屬甘美醫(yī)院肝膽胰二科（云南昆明650011）;

目的探討肝血管瘤切除術(shù)中血流阻斷方法的選擇。方法回顧性分析我院收治的19例肝血管瘤患者的手術(shù)方式。結(jié)果全組均行手術(shù)切除，術(shù)中出血50～1 500 ml（平均312 ml）。 術(shù)中根據(jù)血管瘤所在位置選擇不同肝血流阻斷方法，其中行半肝血流阻斷4例，運用Glisson蒂橫斷式肝切除術(shù)或其分段原理阻斷Glisson系統(tǒng)分支6例，間斷阻斷第一肝門7例，預(yù)置肝上、下下腔靜脈和第一肝門阻斷帶并間斷阻斷第一肝門2例。 術(shù)后5例并發(fā)右側(cè)胸腔積液，均經(jīng)保守治療后好轉(zhuǎn)，手術(shù)并發(fā)癥發(fā)生率為26.3%（5/19）。 術(shù)后住院7～41 d（平均16.9 d），均治愈出院。12例患者獲隨訪，隨訪0.3～2年（平均1.1年），術(shù)前有癥狀的8例患者癥狀均消失，無復(fù)發(fā)，1例殘留肝內(nèi)血管瘤（直徑 lt;2 cm）。結(jié)論肝血管瘤患者肝切除術(shù)中的入肝血流阻斷應(yīng)強調(diào)個體化，根據(jù)腫瘤位置及大小選擇不同的阻斷方法，使患者術(shù)中出血少，術(shù)后恢復(fù)快。
