目的  探討64排增強(qiáng)CT(MDCT)對(duì)進(jìn)展期胃癌(AGC)手術(shù)根治程度的判斷價(jià)值。
方法  分析70例AGC患者M(jìn)DCT增強(qiáng)掃描的影像學(xué)資料,判斷N2(第7、8和9組)淋巴結(jié)轉(zhuǎn)移及臟器受侵情況,并將CT判斷結(jié)果與手術(shù)后病理結(jié)果進(jìn)行比較。
結(jié)果  62例患者接受手術(shù)治療,8例因手術(shù)禁忌證無(wú)法行手術(shù)治療。MDCT術(shù)前判定N2淋巴結(jié)轉(zhuǎn)移的敏感性為92.0%(46/50),特異性為75.0%(9/12),準(zhǔn)確性為88.7%(55/62)。MDCT術(shù)前判定存在N2或以上淋巴結(jié)轉(zhuǎn)移者81.6%(40/49)行D2以上擴(kuò)大根治術(shù); 判定N2淋巴結(jié)無(wú)轉(zhuǎn)移者92.3%(12/13)行標(biāo)準(zhǔn)D2根治術(shù)。
結(jié)論  MDCT對(duì)診斷AGC N2淋巴結(jié)轉(zhuǎn)移及周圍臟器侵犯有重要價(jià)值,對(duì)AGC手術(shù)根治程度的判定有指導(dǎo)作用。
