• 成都市第一人民醫(yī)院肛腸外科（成都 610041）;

目的  探討結(jié)直腸癌患者預(yù)防性使用抗生素與術(shù)后手術(shù)切口感染之間的關(guān)系。
方法  回顧性分析1998年1月至2008年12月期間成都市第一人民醫(yī)院肛腸科專業(yè)組收治880例確診為結(jié)直腸癌的患者的臨床資料，按照抗生素使用時間將患者分為≥24 h組（n＝401）和 lt;24 h組（n＝479），比較2組患者手術(shù)切口感染的發(fā)生率。
結(jié)果  在有切口感染的患者中，手術(shù)時間明顯延長(P=0.04)，出血量更多(P=0.03)，抗生素使用時間更長（P=0.04）。在結(jié)腸癌手術(shù)中，有、無切口感染的患者其手術(shù)時間的差異有統(tǒng)計學(xué)意義（P=0.01）。而直腸癌手術(shù)中，有、無切口感染的患者其手術(shù)時間、出血量以及抗生素使用時間差異無統(tǒng)計學(xué)意義（P gt;0.05）。
結(jié)論  手術(shù)時間越長或術(shù)中出血越多的患者術(shù)后越易發(fā)生切口感染，而延長抗生素使用時間并不能降低手術(shù)切口感染率。
