目的  探討3.0 T 磁共振氫波譜成像(1H-MRS)在定量評價(jià)肝臟脂肪含量中的價(jià)值。
方法  前瞻性納入22例可獲得肝臟標(biāo)本的患者(活體肝移植供體候選者、部分需作肝段/肝葉切除的肝病患者等)作為研究對象,采用點(diǎn)分辨選擇性波譜序列(point resolved selective spectroscopy,PRESS)對研究對象的肝臟行1HMRS檢查,采用SAGE軟件包測定水峰峰值(PW)、脂峰峰值(PL)、水峰峰下面積(AW)及脂峰峰下面積(AL),計(jì)算肝細(xì)胞相對脂肪含量1(relative lipid content one,RLC1)及肝細(xì)胞相對脂肪含量2(relative lipid content two,RLC2)。于MR掃描后當(dāng)日至1周內(nèi)通過手術(shù)獲取肝臟標(biāo)本,并對標(biāo)本進(jìn)行組織學(xué)檢查,對肝臟脂肪含量進(jìn)行分級,并將影像學(xué)數(shù)據(jù)與病理結(jié)果對照研究。
結(jié)果  22例患者中,7例無脂肪肝,11例輕度脂肪肝,4例中重度脂肪肝; 不同病理級別間比較,PL、AL、RLC1及RLC2的差異均有統(tǒng)計(jì)學(xué)意義(P lt;0.05),隨著病理分級升高,對應(yīng)的各指標(biāo)的值也相應(yīng)升高; PL、AL、RLC1及RLC2與脂變細(xì)胞百分含量(proportion of fatty degenerative cells,PFDC)之間存在線性正相關(guān)關(guān)系(P lt;0.05),以RLC1的相關(guān)系數(shù)最高(0.771 3)。
結(jié)論  1H-MRS能夠較精確地反應(yīng)脂肪肝的嚴(yán)重程度,有望替代有創(chuàng)性的肝穿刺活檢。
