目的 觀察肝左區(qū)肝動(dòng)脈解剖結(jié)構(gòu)，模擬肝左外區(qū)活體肝移植動(dòng)脈切取方法。
方法 解剖非肝病死亡之成人甲醛固定尸體肝臟標(biāo)本30例，觀察新鮮成人尸體肝臟鑄型標(biāo)本30例，測(cè)量肝左及左外區(qū)動(dòng)脈長(zhǎng)度、管徑及屬支分布情況。
結(jié)果 左半肝動(dòng)脈的血供來(lái)自肝固有動(dòng)脈、肝左動(dòng)脈、肝中動(dòng)脈，肝外迷走動(dòng)脈支有左膈下動(dòng)脈、胃左動(dòng)脈和胃右動(dòng)脈，并于不同部位發(fā)出后分別進(jìn)入左外區(qū)上、下段。
結(jié)論 左半肝動(dòng)脈主要有5種類型，因此解剖變異較多，左外區(qū)活體取肝前應(yīng)仔細(xì)研究其結(jié)構(gòu)特點(diǎn)，設(shè)計(jì)合理的切取模式； 移植前肝動(dòng)脈需進(jìn)行必要的整形，以便與受體動(dòng)脈吻合。
