每位外科醫(yī)師均會遭遇到外科感染,這是手術(shù)技術(shù)自身所決定了的,外科手術(shù)必然破壞人體宿主抵抗力的第一道防線皮膚和粘膜屏障,使得細(xì)菌得以進(jìn)入體內(nèi)。從外科發(fā)展歷史看,因外科感染而死亡是很常見的,特別是在19世紀(jì)末葉以前,當(dāng)時截肢術(shù)死亡率在25%~90%之間,美國內(nèi)戰(zhàn)時期則為50%左右,這主要是外源性感染所致。但從現(xiàn)代外科學(xué)觀點(diǎn)看,外科感染的內(nèi)涵已發(fā)生了許多重要變化,如醫(yī)院內(nèi)獲得性感染超過了患者原有的感染; 體內(nèi)的內(nèi)源性正常菌群感染多于外源性致病細(xì)菌感染; 外科手術(shù)后的手術(shù)部位感染(surgical site infection,SSI)多于需要外科處理的一般化膿性感染。其中SSI已成為值得關(guān)注的臨床課題。
