為了解急性壞死性胰腺炎(acute necrotizing pancreatitis,ANP)時(shí)血漿內(nèi)皮素(endothelin,ET)的變化及其病理意義,進(jìn)行了前瞻性動(dòng)物實(shí)驗(yàn)。即將SD大鼠隨機(jī)分為3組: 急性壞死性胰腺炎組(ANP組,n=24),經(jīng)膽胰管注入5%?;悄懰徕c(STC,1 ml/kg)制造ANP模型; 假手術(shù)組(SO組,n=24)和血小板激活因子拮抗劑BN50739組(BN組,n=24)。測(cè)定血漿ET及血小板激活因子(platelet activating factor,PAF)水平和胰腺微循環(huán)血流量,并進(jìn)行胰腺組織病理學(xué)評(píng)分。結(jié)果: ANP組在誘發(fā)胰腺炎后胰腺微循環(huán)血流量立即大幅度下降,于3小時(shí)時(shí)有所回升,以后再持續(xù)下降。血漿ET、PAF水平及病理記分均顯著高于SO組。BN組血流顯著改善,血漿ET、PAF水平及病理記分顯著低于ANP組。結(jié)論: 在STC誘發(fā)的大鼠ANP發(fā)病之初,存在胰腺缺血再灌注(I/R)現(xiàn)象,導(dǎo)致內(nèi)皮細(xì)胞損傷,血漿ET及PAF增加。I/R損傷、PAF與ET互相作用,形成惡性循環(huán),加重胰腺病變。
