為了解急性壞死性胰腺炎(acute necrotizing pancreatitis,ANP)時血漿內(nèi)皮素(endothelin,ET)的變化及其病理意義,進行了前瞻性動物實驗。即將SD大鼠隨機分為3組: 急性壞死性胰腺炎組(ANP組,n=24),經(jīng)膽胰管注入5%牛磺膽酸鈉(STC,1 ml/kg)制造ANP模型; 假手術組(SO組,n=24)和血小板激活因子拮抗劑BN50739組(BN組,n=24)。測定血漿ET及血小板激活因子(platelet activating factor,PAF)水平和胰腺微循環(huán)血流量,并進行胰腺組織病理學評分。結(jié)果: ANP組在誘發(fā)胰腺炎后胰腺微循環(huán)血流量立即大幅度下降,于3小時時有所回升,以后再持續(xù)下降。血漿ET、PAF水平及病理記分均顯著高于SO組。BN組血流顯著改善,血漿ET、PAF水平及病理記分顯著低于ANP組。結(jié)論: 在STC誘發(fā)的大鼠ANP發(fā)病之初,存在胰腺缺血再灌注(I/R)現(xiàn)象,導致內(nèi)皮細胞損傷,血漿ET及PAF增加。I/R損傷、PAF與ET互相作用,形成惡性循環(huán),加重胰腺病變。
