為探討腸源性感染的發(fā)生機理,將60只SD大鼠隨機均分成腸梗阻組、環(huán)磷酰胺+腸梗阻組、環(huán)磷酰胺組和假手術(shù)組4組。回腸機械性腸梗阻24小時后取各組鼠腹腔液、門靜脈血、心臟血、腸系膜淋巴結(jié)(MLN)和回腸末段內(nèi)容物作細菌定量分析,同時測定門靜脈血內(nèi)毒素含量。結(jié)果顯示:腸梗阻24小時即導(dǎo)致門靜脈內(nèi)毒素血癥,腸內(nèi)G-桿菌大量繁殖,腸菌移位于MLN中。免疫抑制劑環(huán)磷酰胺不僅增加腸梗阻時腸菌移位的發(fā)生率(100%∶80%),且使更多的腸菌存活于MLN中并向體循環(huán)內(nèi)播散。由此表明,在免疫抑制狀態(tài)下腸菌經(jīng)淋巴途徑侵入腸外組織,在腸源性感染的發(fā)生中起著重要作用。
