目的 探討肺隱球菌病( PC) 的臨床特點、影像學表現(xiàn)、診斷和治療, 提高對該病的認識。方法 回顧性分析解放軍第八一醫(yī)院、上海長海醫(yī)院和上海長征醫(yī)院2000 年1 月至2009 年1 月收治的34 例PC 的臨床資料。結(jié)果 34 例PC 中, 男24 例, 女10 例; 年齡3 ～72 歲, 平均( 40. 6 ± 13. 5) 歲; 有基礎(chǔ)疾病12 例( 35. 3% ) ; 有癥狀28 例( 82. 4% ) , 主要為發(fā)熱( 20 例) , 咳嗽、咳痰( 22 例) , 胸痛( 8 例) , 胸悶( 5 例) , 咯血( 4 例) 。全身播散性PC 7 例( 20. 6% ) , 其中合并隱球菌腦膜炎3 例, 同時合并隱球菌腦膜炎、敗血癥3 例, 合并第三胸椎隱球菌病1 例。影像學表現(xiàn)呈多樣性與非特異性, 分為肺結(jié)節(jié)、腫塊型( 17 例) , 肺炎型( 10 例) , 混合型( 6 例) , 彌漫性粟粒影( 1 例) 。組織病理確診27 例, 隱球菌培養(yǎng)或涂片檢查陽性10 例。單純手術(shù)治療6 例, 單純藥物治療21 例, 手術(shù)+ 藥物治療6 例, 放棄治療1 例?？拐婢幬锆煶? 周～2 年, 住院死亡1 例。隨訪25 例治愈, 8 例失訪。結(jié)論 PC 的臨床和影像學表現(xiàn)不典型, 臨床常易誤診, 診斷依賴于病理學及微生物學檢查。
