目的 研究聯(lián)合檢測微衛(wèi)星不穩(wěn)定性( MSI) 和p16 基因啟動子甲基化在早期肺癌中的診斷價(jià)值。方法 對肺癌、癌前病變和正常肺組織采用PCR 單鏈長度分析法檢測MSI, 采用甲基化特異PCR 法檢測p16 甲基化。結(jié)果 癌前病變組MSI 的發(fā)生率顯著高于肺癌組( P  lt;0. 05) 和正常肺組織組( P  lt;0. 01) , 肺癌組MSI 的發(fā)生率顯著高于正常肺組織組( P  lt;0. 01) ; 肺癌組p16 甲基化的發(fā)生率顯著高于癌前病變組( P  lt;0. 01) 和正常肺組織組( P  lt;0. 01) , 癌前病變組p16 甲基化的發(fā)生率顯著高于正常肺組織組( P  lt;0. 01) ; 聯(lián)合檢測MSI 和p16 甲基化的敏感性顯著高于單一檢測MSI( P  lt;0. 01) 和p16 甲基化( P  lt;0. 05) ; 聯(lián)合檢測法與單一檢測MSI 和單一檢測p16 甲基化的特異性比較,差異無顯著性意義( P  gt;0. 05) 。結(jié)論 聯(lián)合檢測MSI 和p16 甲基化可以顯著提高早期肺癌診斷的敏感性同時(shí)不降低其特異性, 值得臨床推廣。
