目的  評(píng)價(jià)綜合治療( 有創(chuàng)無創(chuàng)序貫機(jī)械通氣, 纖支鏡吸痰, 聯(lián)合霧化吸入異丙托溴銨、沙丁胺醇和布地奈德) 在老年COPD 所致急性呼吸衰竭患者的臨床療效。方法  納入2006 年7月至2009 年7 月間ICU收治的老年COPD 合并急性呼吸衰竭患者59 例, 隨機(jī)分為無創(chuàng)通氣組( 20例) 、有創(chuàng)無創(chuàng)序貫通氣組( 20 例) 和綜合治療組( 19 例) , 在常規(guī)治療基礎(chǔ)上予以相應(yīng)干預(yù)。監(jiān)測患者治療0 h、3 h、3 d、12 d 的APACHEⅡ評(píng)分、臨床肺部感染評(píng)分( CPIS) 、動(dòng)脈血?dú)?、心率、血? 以及肺部感染控制窗出現(xiàn)時(shí)間、機(jī)械通氣時(shí)間、ICU住院時(shí)間、總住院時(shí)間、呼吸機(jī)相關(guān)性肺炎( VAP) 患病率、病死率等。結(jié)果  三組患者的APACHEⅡ評(píng)分、CPIS評(píng)分、呼吸頻率、心率、PaCO2 、白細(xì)胞計(jì)數(shù)隨治療時(shí)間的延長而逐漸降低( P  lt;0. 05) , pH 值、PaO2 隨治療時(shí)間的延長而逐漸增高( P  lt;0. 05) 。三組間呼吸頻率、心率、PaCO2、PaO2 在治療3 d 及12 d 時(shí)有顯著差異( P  lt; 0. 05) , 序貫組優(yōu)于無創(chuàng)組,綜合組優(yōu)于序貫組。三組肺部感染控制窗和VAP 出現(xiàn)時(shí)間無顯著差異( P  gt;0. 05) 。序貫組總機(jī)械通氣時(shí)間、ICU住院時(shí)間和總住院時(shí)間明顯短于無創(chuàng)組( P  lt; 0. 05) , 綜合組總機(jī)械通氣時(shí)間、有創(chuàng)通氣時(shí)間、ICU住院時(shí)間和總住院時(shí)間短于序貫組( P  lt;0. 05) 。序貫組VAP患病率高于無創(chuàng)組, 住院病死率低于無創(chuàng)組。綜合組VAP 患病率低于序貫組但仍高于無創(chuàng)組( P  lt;0. 05) , 住院病死率低于序貫組( P  lt;0. 05) 。結(jié)論  對(duì)老年COPD 合并急性呼吸衰竭患者, 有創(chuàng)無創(chuàng)序貫通氣明顯優(yōu)于單用無創(chuàng)通氣, 且盡早給予綜合治療( 有創(chuàng)無創(chuàng)序貫通氣、纖支鏡吸痰、聯(lián)合霧化吸入) 可縮短機(jī)械通氣時(shí)間和住院時(shí)間, 降低VAP 患病率和病死率。
