目的 探索主動(dòng)脈瓣病變合并冠心病患者的臨床特點(diǎn),以提高臨床治療的針對(duì)性?!》椒ā』仡櫺苑治錾蜿栜妳^(qū)總醫(yī)院2010年1~10月期間78例50歲以上行主動(dòng)脈瓣置換術(shù)患者的臨床資料,根據(jù)是否合并冠心病,將患者分為兩組:合并冠心病組(病例組),20例,男19例、女1例,年齡(64.6±8.3) 歲;單純主動(dòng)脈瓣置換術(shù)組(對(duì)照組),58例,其中男28例、女30例,年齡(58.7±6.2)歲。通過單因素分析和多因素分析,探索主動(dòng)脈瓣置換術(shù)患者合并冠心病的臨床特點(diǎn)。 結(jié)果 單因素分析結(jié)果顯示,病例組年齡(P=0.000)、男性 (P=0.004)和糖尿病史(P=0.004)、主動(dòng)脈瓣反流(P=0.034)、心瓣膜病變(P=0.028)、呼吸機(jī)輔助時(shí)間(P=0.008)均明顯高于對(duì)照組,且差異有統(tǒng)計(jì)學(xué)意義。多因素分析結(jié)果顯示,主動(dòng)脈瓣置換術(shù)患者合并冠心病的獨(dú)立臨床特點(diǎn)包括男性、年齡較大、術(shù)后呼吸機(jī)輔助時(shí)間明顯延長(zhǎng)、術(shù)后6個(gè)月時(shí)的肺動(dòng)脈收縮壓(PASP)較對(duì)照組明顯下降、術(shù)前右心室舒張功能明顯減低。 結(jié)論 單純主動(dòng)脈瓣病變男性患者若術(shù)前伴有右心室舒張功能降低和/或主動(dòng)脈瓣呈三葉的退行性病變,應(yīng)將冠狀動(dòng)脈病變篩選年齡適當(dāng)放寬;單純行主動(dòng)脈瓣置換術(shù)的患者應(yīng)較長(zhǎng)時(shí)間地服用逆轉(zhuǎn)左心室重構(gòu)的藥物,以避免惡性心律失常的發(fā)生。
