• 武漢亞洲心臟病醫(yī)院　心臟重癥監(jiān)護室，武漢　430022;

目的　觀察并評價馬方綜合征（Marfan syndrome，MFS）并發(fā)A型主動脈夾層（type A aortic dissection，AAD）的手術(shù)療效。　方法　回顧性分析2006年1月至2012年1月武漢亞洲心臟病醫(yī)院收治44例MFS并發(fā)AAD患者的臨床資料，其中男31例，女13例；年齡12～54 （33.0±9.8）歲。23例在AAD發(fā)病后不同疾病階段施行外科手術(shù)治療，均行Bentall手術(shù)； 21例因經(jīng)濟或其它原因未采取手術(shù)治療。采用含時變協(xié)變量的COX回歸分析，分別以主要終點事件、主要＋次要終點事件作為結(jié)局變量對相關(guān)因素進行分析，觀察手術(shù)患者術(shù)后手術(shù)療效和生活質(zhì)量，并與未手術(shù)患者的隨訪資料進行對比?！〗Y(jié)果　術(shù)后有1例手術(shù)患者死于多器官功能衰竭，其余22例手術(shù)均成功。隨訪43例，隨訪時間1～75個月。22例手術(shù)患者術(shù)后3年生存率為95.7%，隨訪期間生活質(zhì)量良好。未行手術(shù)治療的21例患者生存狀況及生活質(zhì)量較差，13例（61.9%）死亡，主要死亡原因：急性心臟壓塞、主動脈夾層破裂、急性心肌梗死并發(fā)心源性休克等；發(fā)病后3年生存率僅為31.7%。采用含時變協(xié)變量COX回歸模型分析校正手術(shù)時間不一產(chǎn)生影響后的結(jié)果顯示，手術(shù)治療患者與未手術(shù)治療患者相比較，其臨床療效的差異有統(tǒng)計學意義（T_COV_的OR值=0.088，P=0.028），手術(shù)治療死亡風險僅為非手術(shù)治療的8.8% （P＜0.05）。　結(jié)論　MFS并發(fā)AAD預(yù)后兇險，應(yīng)盡早外科手術(shù)，不論是急性期還是慢性期均可獲益，療效可靠，隨訪結(jié)果令人滿意。
