摘要:目的 總結(jié)非原位主動(dòng)脈瓣置換術(shù)( AVR)的方法和療效。方法 2008年 3月至 2010年 12月第二軍醫(yī)大學(xué)長(zhǎng)海醫(yī)院收治 5例主動(dòng)脈瓣環(huán)嚴(yán)重毀損患者,男 4例,女 1例;平均年齡 46.3(38~53)歲。 AVR術(shù)后并發(fā)嚴(yán)重瓣周漏再次手術(shù) 4例(白塞氏病 2例、大動(dòng)脈炎 2例),嚴(yán)重主動(dòng)脈瓣感染性心內(nèi)膜炎 1例。術(shù)中發(fā)現(xiàn)主動(dòng)脈瓣環(huán)嚴(yán)重毀損,無(wú)法施行常規(guī) AVR,改行非原位 AVR,即將人工主動(dòng)脈瓣環(huán)固定的位置提高到毀損的主動(dòng)脈瓣環(huán)以上,固定人工瓣環(huán)的縫線(xiàn)從主動(dòng)脈腔外進(jìn)針、腔內(nèi)出針。必要時(shí)行冠狀動(dòng)脈旁路移植術(shù)( CABG)。結(jié)果 5例手術(shù)患者均順利康復(fù)出院,無(wú)出血再次手術(shù)或其它并發(fā)癥。術(shù)后 5例患者均獲得隨訪(fǎng),生存良好,心功能分級(jí)(NYHA)Ⅰ~Ⅱ級(jí);分別于術(shù)后 6個(gè)月( 4例)、 1年(2例)和 3年(1例)行主動(dòng)脈心臟三維 CT成像( CTA)檢查,均未見(jiàn)有主動(dòng)脈根部瘤或假性動(dòng)脈瘤形成,顯示左心室流出道延長(zhǎng),左、右冠狀動(dòng)脈形態(tài)良好。心臟超聲心動(dòng)圖檢查均未發(fā)現(xiàn)有瓣周漏。結(jié)論 對(duì)主動(dòng)脈瓣環(huán)毀損嚴(yán)重的患者采用非原位 AVR,人工瓣膜固定可靠,可以防止術(shù)后并發(fā)瓣周漏或根部假性動(dòng)脈瘤。
