目的探討改良 Carpentier法矯治三尖瓣下移畸形( Ebstein畸形)的手術(shù)方法,總結(jié)其臨床經(jīng)驗(yàn)。方法 回顧性分析 2006年 6月至 2010年 8月安徽醫(yī)科大學(xué)第一附屬醫(yī)院 13例 Ebstein畸形患者手術(shù)治療的臨床資料,其
中男 6例,女 7例;年齡( 26.8±13.5)歲。所有患者均采用改良 Carpentier法矯治,手術(shù)方法包括:切除部分房化右心室,折疊環(huán)縮三尖瓣瓣環(huán)至適當(dāng)?shù)拇笮。米泽w心包片擴(kuò)大后瓣葉 /隔瓣葉,移位縫合切下的部分前瓣葉(向內(nèi)旋轉(zhuǎn)后)、后瓣及隔瓣葉縫至正常的瓣環(huán)水平,移位相應(yīng)的乳頭肌、腱索,擴(kuò)大修復(fù)瓣葉,加固瓣環(huán),并矯正其他合并的心血管畸形。結(jié)果所有患者術(shù)后均恢復(fù)順利,無死亡。超聲心動(dòng)圖提示:三尖瓣功能良好,三尖瓣瓣葉均在正常位置;三尖瓣有輕度至中度反流 3例,無反流或有輕微反流 10例。心功能分級(jí)( NYHA)I~Ⅱ級(jí)。所有患者均得到隨訪,隨訪時(shí)間 3~15個(gè)月,平均 8個(gè)月。術(shù)后 3個(gè)月和 1年分別復(fù)查超聲心動(dòng)圖提示:三尖瓣啟閉及右心室功能良好,三尖瓣無明顯反流 12例,1例出院時(shí)三尖瓣有中度反流患者轉(zhuǎn)為輕度反流。所有患者恢復(fù)正常的生活或工作。結(jié)論改良 Carpentier法矯治 Ebstein畸形有較好的近期療效,該手術(shù)方法切除無功能的房化右心室后重新縫合塑形,有利于右心室形態(tài)重建和功能恢復(fù),通過自體心包片擴(kuò)大瓣葉、充分的瓣葉移位及后乳頭肌、腱索移位重建可達(dá)到良好的瓣膜成形效果。
