目的 探討肺移植和肺減容手術(shù)( LVRS)治療終末期肺氣腫患者的臨床療效及衛(wèi)生經(jīng)濟(jì)學(xué)指標(biāo),為臨床選擇治療方案提供參考。方法 回顧性分析自 2002年 9月至 2008年 8月無錫市人民醫(yī)院 61例終末期肺氣腫患者行 LVRS和肺移植術(shù)治療的臨床資料,根據(jù)手術(shù)方式不同將 61例患者分為 3組,LVRS組:39例,行單側(cè)肺減容術(shù);單肺移植術(shù)( SLTx)組: 14例,行 SLTx;雙肺移植術(shù)( BLTx)組: 8例,行 BLTx。于圍術(shù)期、術(shù)后 6個月、 1年和 3年住院復(fù)查肺功能、血氣分析指標(biāo)、 6 min步行距離(6-MWD)的改變,觀察 1年、3年生存率。并通過成本 -效用分析進(jìn)行衛(wèi)生經(jīng)濟(jì)學(xué)評價。結(jié)果 術(shù)后 6個月、1年和 3年 LVRS組、SLTx組和 BLTx組 FEV1.0較術(shù)前分別改善 75%、83%和 49%,176% 、162% 和 100%,260% 、280% 和 198%;LVRS組、SLTx組和 BLTx組 FVC分別較術(shù)前改善 21%、41%和 40%,68%、73%和 55%,82%、79%和 89%;LVRS組、SLTx組和 BLTx組 6-MWD分別較術(shù)前增加 75%、136%和 111% ,513% 、677% 和 608%,762% 、880%和 741%。LVRS組、SLTx組和 BLTx組患者 1年、3年生存率分別為 74.40%和 58.90%,85.80%和 64.30%,62.50%和 50.00%。SLTx組隨訪 3年的成本 -效用比高于 BLTx組(1 668.00 vs.1 168.55,P< 0.05)和 LVRS組(1 668.00 vs. 549.46, P< 0.05)。結(jié)論 SLTx、
BLTx組術(shù)后各項功能指標(biāo)較 LVRS組明顯改善。 3年內(nèi)成本 -效用分析結(jié)果表明, LVRS組更經(jīng)濟(jì)、實惠。隨著醫(yī)療技術(shù)的進(jìn)步、移植醫(yī)療費用的降低、免疫抑制劑的國產(chǎn)化價格降低,肺移植總費用也隨之降低,肺移植術(shù)將成為終末期肺氣腫患者首選的外科治療方法。
