目的探討獲得性魯?shù)前秃站C合征的手術(shù)時(shí)機(jī)及治療方法，以改善預(yù)后。方法回顧性分析 2000年 1月至 2009年 12月北京安貞醫(yī)院 16例獲得性魯?shù)前秃站C合征患者手術(shù)治療的臨床資料，其中男 9例，女 7例；年齡（39.45±10.23）歲。所有患者在氣管內(nèi)插管靜脈復(fù)合全身麻醉體外循環(huán)下施行手術(shù)，經(jīng)胸骨正中切口徑路進(jìn)胸行二尖瓣置換術(shù)＋房間隔缺損修補(bǔ)術(shù)。術(shù)后觀察圍術(shù)期死亡和并發(fā)癥發(fā)生情況。隨訪期間采用超聲心動(dòng)圖觀察有否瓣周漏和房間隔水平殘余分流、心室的大小、肺動(dòng)脈壓的改變和心功能的改善情況等。結(jié)果術(shù)后死亡 2例（12.5%），其中 1例死于腎功能衰竭，另 1例死于肺部感染。1例術(shù)中不能脫離體外循環(huán)，應(yīng)用體外膜式氧合（ECMO）輔助 3 d，順利撤除，痊愈出院； 1例應(yīng)用連續(xù)性腎臟替代治療（ CRRT）11 d，逐漸恢復(fù)； 1例術(shù)后 6 d做氣管切開，術(shù)后 23 d脫離呼吸機(jī)，自主呼吸，痊愈出院。隨訪 14例，隨訪率 100%，隨訪時(shí)間 6個(gè)月～ 5年，術(shù)后 6個(gè)月復(fù)查超聲心動(dòng)圖提示：左心室舒張期末內(nèi)徑（ 42.1±5.7）mm，無瓣周漏和房間隔水平殘余分流，射血分?jǐn)?shù) 67.4%±6.7%，估測(cè)肺動(dòng)脈壓（ 23.4±5.4）mm Hg。心功能分級(jí)（ NYHA）Ⅱ級(jí) 12例，Ⅲ級(jí) 2例。結(jié)論對(duì)手術(shù)時(shí)機(jī)的選擇，只要患者左心室舒張期末容積指數(shù)提示可以耐受手術(shù)，就應(yīng)該盡早施行手術(shù)治療。對(duì)術(shù)前判定脫離體外循環(huán)困難的患者，建議直接應(yīng)用 ECMO輔助。
