目的  總結(jié)保留二尖瓣后瓣及瓣下結(jié)構(gòu)二尖瓣置換術(shù)( MVR)的經(jīng)驗(yàn),評(píng)價(jià)其臨床效果。方法  回顧性分析首都醫(yī)科大學(xué)北京安貞醫(yī)院 2006年 1月至 2011年 3月行 MVR患者 1 035例的臨床資料,其中男 562例,女 473例;年齡 37~ 78(53.84±13.13)歲。風(fēng)濕性心臟瓣膜病 712例,退行性瓣膜病 323例;二尖瓣狹窄為主 389例,二尖瓣關(guān)閉不全為主 646例;均排除冠狀動(dòng)脈疾病。不保留后瓣及瓣下結(jié)構(gòu)的 MVR(不保留組) 457例,保留后瓣及瓣下結(jié)構(gòu)的 MVR(保留組) 578例,兩組患者術(shù)前情況差異無(wú)統(tǒng)計(jì)學(xué)意義。分析比較兩組患者手術(shù)后并發(fā)癥、死亡率,以及左心室大小與功能。結(jié)果  不保留組與保留組患者的死亡率( 2.63%vs.1.21%,P=0.091)及并發(fā)癥發(fā)生率( 8.53% vs. 7.44%,P=0.519)差異無(wú)統(tǒng)計(jì)學(xué)意義,但不保留組左心室破裂發(fā)生率高于保留組(1.09% vs. 0.00%,P=0.012);術(shù)后 6個(gè)月心臟超聲心動(dòng)圖提示左心室舒張期末內(nèi)徑( LVEDD)較術(shù)前縮小,但兩組差異無(wú)統(tǒng)計(jì)學(xué)意義;兩組患者左心室射血分?jǐn)?shù)(LVEF)均較術(shù)前提高,保留組中二尖瓣關(guān)閉不全為主患者的 LVEF優(yōu)于不保留組( 56.00%±3.47% vs. 53.00%±3.13%,P=0.000),兩組二尖瓣狹窄為主的患者中 LVEF差異無(wú)統(tǒng)計(jì)學(xué)意義(57.00%±5.58% vs. 56.00%±4.79%,P=0.066)。結(jié)論  保留二尖瓣后瓣及瓣下結(jié)構(gòu)的 MVR安全有效,可以減少術(shù)后左心室破裂的發(fā)生并改善術(shù)后心功能。
