目的?總結在動脈轉位術(arterial switch operation,ASO)中采用雙活瓣延長技術做冠狀動脈移植治療完全型大動脈轉位(D-transposition of the great arteries,D-TGA)和Taussig-Bing畸形的臨床經驗,探討該技術的手術方法和手術適應證。?方法?回顧性分析2006年1月至2011年6月上海交通大學醫(yī)學院附屬上海兒童醫(yī)學中心21例合并復雜冠狀動脈畸形的D-TGA、L-TGA或Taussig-Bing患者[男13例,女8例;年齡(110.0±84.5) d;體重(5.4±4.2) kg]行ASO治療,術中采用雙活瓣延長技術做冠狀動脈移植的臨床資料。所有患者均為右冠狀動脈主干或粗大右心室圓錐支發(fā)自主動脈左側或右側瓣竇,并異常前繞于主動脈根部行走。雙活瓣延長技術包括:從主動脈剪取較長的冠狀動脈紐片做為活瓣;剪取肺動脈(新主動脈)另一帶蒂活瓣向冠狀動脈紐片方向做等距離延長;活瓣與紐片的邊緣互相縫合形成冠狀動脈的延長管道。?結果?無住院死亡,術后呼吸機使用時間(101.6±53.6) h,監(jiān)護時間(9.5±4.9)d。術后發(fā)生低心排血量9例,肺動脈高壓危象2例,肺部感染6例,急性腎功能衰竭2例,均經相應的治療治愈出院。11例患者術后延遲關胸。隨訪17例,隨訪時間2個月~5年,隨訪期間患者生長發(fā)育均明顯改善,無缺血性心電圖改變。1例肺動脈瓣上狹窄患者于ASO術后2年再次行手術修補。?結論 雙活瓣延長技術做冠狀動脈移植在復雜ASO中能有效減少因冠狀動脈畸形導致的手術死亡,尤其適合于二期ASO以及右冠狀動脈主干或粗大圓錐支發(fā)自左側或右側瓣竇,并沿主動脈異常前繞的患者。
