目的 評價中國冠狀動脈旁路移植手術(shù)風(fēng)險評估系統(tǒng)(Sino System for Coronary Operative Risk Evaluation,SinoSCORE) 對華西醫(yī)院(本中心)成人心臟手術(shù)后院內(nèi)死亡風(fēng)險的預(yù)測價值?!》椒ā∵B續(xù)納入2010年1月至2012年5月進入中國成人心臟外科數(shù)據(jù)庫、本中心2 088例行心臟手術(shù)患者的臨床資料,比較本中心與中國成人心臟外科數(shù)據(jù)庫中所有患者術(shù)前危險因素的差異。計算每例患者的SinoSCORE累計積分,評價SinoSCORE預(yù)測本中心患者院內(nèi)死亡風(fēng)險的鑒別度和校準度?!〗Y(jié)果 本中心2 088例患者中行冠狀動脈旁路移植術(shù)(CABG) 168例(8.05%),心瓣膜手術(shù)1 884例(90.23%),其它手術(shù)36例(1.72%)。本中心患者的高脂血癥、腦卒中、心血管手術(shù)史、腎臟疾病等與中國成人心臟外科數(shù)據(jù)庫中全部患者的差異有統(tǒng)計學(xué)意義。本中心患者實際術(shù)后院內(nèi)病死率為2.25% (47/2 088),SinoSCORE預(yù)測院內(nèi)病死率為2.35% (49/2 088)[95% CI (2.18,2.47)]。SinoSCORE預(yù)測本中心患者術(shù)后院內(nèi)病死率的校準度(χ2=3.164, P=0.582)和鑒別度[受試者工作特征曲線下面積0.751,95% CI (0.719,0.924)] 均較好。 結(jié)論 SinoSCORE對中國西南地區(qū)成人心臟手術(shù)后院內(nèi)死亡風(fēng)險的預(yù)測雖高估風(fēng)險,但仍適用。
