目的 探討胸內(nèi)食管吻合口隱匿瘺的診斷治療方法。 方法 回顧性分析2006年1月至2010年12月青島大學(xué)醫(yī)學(xué)院附屬醫(yī)院32例食管、賁門(mén)癌患者行胸內(nèi)胃食管吻合術(shù)后出現(xiàn)膿毒血癥表現(xiàn),并先后行水溶性造影劑食管X線造影+胸部CT掃描患者的臨床資料。32例均無(wú)造影劑滲漏,但CT顯示吻合口周?chē)霈F(xiàn)小氣泡、包裹性積液。其中15例按“隱匿瘺”處理,給予禁食、廣譜抗生素、延長(zhǎng)胃腸減壓時(shí)間、鼻腸管腸內(nèi)營(yíng)養(yǎng)等治療;另17例按非吻合口瘺相關(guān)感染處理,僅應(yīng)用高效廣譜抗生素?!〗Y(jié)果 32例患者中按吻合口瘺處理的15例患者口服水溶性造影劑食管X線造影+胸部CT掃描顯示均未發(fā)展為吻合口瘺(0%,0/15);而未按吻合口瘺處理的17例患者中有14例發(fā)展為吻合口瘺(82.4%,14/17),其中2例發(fā)生吻合口主動(dòng)脈瘺死亡,3例并發(fā)多器官功能衰竭死亡?!〗Y(jié)論 口服水溶性造影劑食管X線造影+胸部CT掃描顯示吻合口周?chē)行馀荨⒉灰?guī)則包裹性積液為“隱匿瘺”的特異性征象,應(yīng)按吻合口瘺處理。
