目的 探討不宜單獨(dú)行腔內(nèi)隔絕治療、累及弓部的主動(dòng)脈夾層雜交手術(shù)治療方法及其療效。 方法 回顧性分析2008年11月至2011年8月成都軍區(qū)總醫(yī)院15例累及弓部的主動(dòng)脈夾層患者行雜交手術(shù)治療的臨床資料,其中男10例,女5例;年齡51~72 (58.2±7.2)歲。Stanford A型主動(dòng)脈夾層4例,B型主動(dòng)脈夾層11例,病變均累及主動(dòng)脈弓。采用胸骨正中切口或加頸部切口行升主動(dòng)脈至頭臂動(dòng)脈旁路移植、單純頸部切口行頭臂動(dòng)脈間旁路移植,然后行股動(dòng)脈切口逆行主動(dòng)脈腔內(nèi)覆膜支架植入。術(shù)后即刻行數(shù)字減影血管造影(DSA),術(shù)后3個(gè)月、術(shù)后1年和2年分別隨訪CT造影資料,觀察支架和人工血管通暢情況?!〗Y(jié)果 所有患者均成功完成手術(shù),并植入覆膜支架。術(shù)中血管造影證實(shí)支架植入定位準(zhǔn)確,支架無(wú)明顯內(nèi)漏和移位。主動(dòng)脈夾層真腔血流恢復(fù)正常,旁路血管血流通暢,圍術(shù)期無(wú)死亡和嚴(yán)重并發(fā)癥發(fā)生。隨訪15例,隨訪時(shí)間3~20 (12.0±4.1)個(gè)月,所有患者均生存,恢復(fù)正常生活。術(shù)后3個(gè)月及術(shù)后1年、2年復(fù)查主動(dòng)脈增強(qiáng)CT示:支架無(wú)移位和內(nèi)漏,支架內(nèi)及人工血管旁路血流通暢,未見(jiàn)腦部和肢體缺血征象?!〗Y(jié)論 累及弓部的主動(dòng)脈夾層可根據(jù)受累部位和程度采用不同的雜交手術(shù)方法,安全、有效,能明顯減輕患者的創(chuàng)傷和痛苦,該方法擴(kuò)大了介入覆膜支架腔內(nèi)治療的適應(yīng)證,但遠(yuǎn)期療效有待進(jìn)一步觀察。
