目的 評(píng)價(jià)使用保留主動(dòng)脈瓣主動(dòng)脈根部置換術(shù)治療主動(dòng)脈根部瘤的臨床應(yīng)用療效。?方法 2001年2月至2010年9月阜外心血管病醫(yī)院對(duì)60例主動(dòng)脈根部瘤患者行保留主動(dòng)脈瓣主動(dòng)脈根部置換術(shù),其中男44例,女16例;年齡9~64 (37.2±13.0)歲。主動(dòng)脈夾層15例,升主動(dòng)脈瘤10例,馬方綜合征25例,主動(dòng)脈瓣二瓣化畸形2例。行主動(dòng)脈根部重建術(shù)53例,主動(dòng)脈瓣瓣葉再植術(shù)7例。比較術(shù)前、術(shù)后心功能及主動(dòng)脈瓣反流程度等指標(biāo)。?結(jié)果 全組中無(wú)手術(shù)死亡和嚴(yán)重并發(fā)癥發(fā)生,呼吸機(jī)使用時(shí)間中位數(shù)為13 (2~1 110) h,住ICU時(shí)間1~18(2.7±2.5) d。術(shù)后復(fù)查超聲心動(dòng)圖提示:主動(dòng)脈瓣反流程度均明顯減輕,僅3例為中大量反流,其余無(wú)反流或微少量反流。術(shù)后隨訪2~122 (61.5±35.9)個(gè)月,隨訪56例,失訪4例,隨訪期間死亡9例,生存率83.9% (47/56)。2例因主動(dòng)脈瓣反流分別于術(shù)后13個(gè)月和14個(gè)月再次入院行主動(dòng)脈瓣置換術(shù)。47例患者心功能較術(shù)前明顯改善,心功能Ⅰ級(jí)35例(74.4%),Ⅱ級(jí)8例(17.0%)。免于主動(dòng)脈瓣中重度反流40例(85.1%)。?結(jié)論 保留瓣葉的主動(dòng)脈根部置換術(shù)治療主動(dòng)脈根部瘤的遠(yuǎn)期療效滿意,瓣膜相關(guān)并發(fā)癥發(fā)生率低。
