目的 總結(jié)低左心室射血分?jǐn)?shù)(≤40%)冠心病患者行非體外循環(huán)冠狀動脈旁路移植術(shù)(OPCAB)的手術(shù)療效及預(yù)后相關(guān)因素。 方法 回顧性分析2001~2004年在北京大學(xué)人民醫(yī)院行OPCAB的低射血分?jǐn)?shù)患者順利出院63例的隨訪臨床資料,男48例,女15例;平均年齡(65.1±9.2)歲,左心室射血分?jǐn)?shù)為33.8%±5.0%,對其長期隨訪,使用Kaplan-Meier法繪制生存曲線,采用單因素對數(shù)秩檢驗(log-rank test)和Cox回歸模型分析影響患者長期預(yù)后的危險因素。 結(jié)果 隨訪時間3~107 (71.3±24.4)個月,失訪6例(9.5%)。隨訪期內(nèi)死亡19例(30.2%),其中與心臟相關(guān)死亡10例(15.9%)。術(shù)后1年、3年、5年、8年的生存率分別為96.7% (61)、94.9% (60)、85.9% (55)、77.2% (53)。單因素分析表明,左心室射血分?jǐn)?shù)≤30%、急性心肌梗死<30 d(急性心肌梗死發(fā)生30 d內(nèi)即接受手術(shù))是影響患者遠(yuǎn)期生存率的潛在危險因素(P<0.05)。Cox回歸分析表明,左心室射血分?jǐn)?shù)≤30% (RR=4.662,P<0.05)及心肌梗死<30 d (RR=5.544,P<0.05)是擇期OPCAB患者術(shù)后發(fā)生心臟相關(guān)死亡的獨立危險因素?!〗Y(jié)論 低左心室射血分?jǐn)?shù)冠心病患者行OPCAB治療效果良好。左心室射血分?jǐn)?shù)≤30%及急性心肌梗死<30 d是擇期OPCAB患者術(shù)后發(fā)生心臟相關(guān)死亡的獨立危險因素。
