目的 探討急性心肌梗死(AMI)后患者近期行非體外循環(huán)冠狀動脈旁路移植術(shù)(OPCAB)的臨床療效和圍術(shù)期處理方法?!》椒ā?006年1月至2010年3月,上海交通大學(xué)醫(yī)學(xué)院附屬仁濟(jì)醫(yī)院連續(xù)239例患者在發(fā)生AMI后14~27 (20.55±3.91) d行OPCAB (AMI組),術(shù)前磷酸肌酸激酶同工酶 (CK-MB,15.82±6.24) U/L,心肌肌鈣蛋白Ⅰ(cTnI,0.07±0.04) ng/ml;將同期406例無心肌梗死史的OPCAB患者作為對照(對照組);比較、分析兩組患者的臨床資料。?結(jié)果?術(shù)后30 d AMI組死亡率為2.51% (6/239),其中死于循環(huán)衰竭4例,主動脈內(nèi)球囊反搏(IABP)導(dǎo)致下肢缺血、壞死1例,肺部感染合并休克1例。AMI組患者術(shù)后應(yīng)用多巴胺明顯多于對照組(61.51% vs.37.44%,P=0.001),術(shù)中/術(shù)后行IABP亦多于對照組,但差異無統(tǒng)計學(xué)意義 (P>0.05)。AMI組術(shù)后引流量及輸注紅細(xì)胞懸液量均多于對照組 [(385.18±93.22) ml vs. (316.41±70.05 )ml,P=0.022; (373.68±69.54) ml vs. (289.78±43.33) ml,P=0.005], 但兩組二次開胸止血率差異無統(tǒng)計學(xué)意義(P>0.05)。兩組術(shù)后新發(fā)心房顫動發(fā)生率差異無統(tǒng)計學(xué)意義(P>0.05);AMI組急性腎損傷發(fā)生率高于對照組(13.81% vs.8.62%,P=0.038)。AMI組術(shù)后30 d死亡率高于對照組 (2.51% vs.1.48%),但差異無統(tǒng)計學(xué)意義(P>0.05)。兩組住ICU時間[(2.01±0.95) d vs. (1.78±0.98) d]和術(shù)后住院時間[(10.33±4.16) d vs. (9.89±4.52) d]差異均無統(tǒng)計學(xué)意義 (P>0.05)。AMI 組隨訪211例(88.28%),隨訪時間(2.89±1.02)年;失訪28例(11.72%)。 隨訪期間死亡25例,其中心源性死亡14例;1年生存率為97.63%,5年生存率88.15%?!〗Y(jié)論 在AMI發(fā)生2~4周、待CK-MB和cTnI水平恢復(fù)至正常范圍后,行OPCAB是相對安全的。
