目的 評價牛頸靜脈帶瓣補片應用于重建右心室流出道的近期效果?!》椒ā』仡櫺苑治?009年5月至2010年3月武漢亞洲心臟病醫(yī)院行牛頸靜脈帶瓣補片重建右心室流出道60例患者的臨床資料,男42例,女18例;年齡5個月~33 (6.2±8.9)歲,其中34例<1歲;體重(27.5±24.0) kg。術前臨床診斷為法洛四聯(lián)癥38例,右心室雙出口合并肺動脈狹窄22例。所有患者均行一期完全修復術,有4例因為側枝循環(huán)較多,術前采取介入側枝封堵。全組患者肺動脈瓣瓣環(huán)直徑均小于正常值2個標準差,均采取跨瓣環(huán)補片重建右心室流出道。隨訪時間18~26(21.2±4.6)個月?!〗Y果 手術無死亡,也無因流出道嚴重狹窄和嚴重肺動脈瓣反流需再次手術者。有3例患者術后早期因為側枝循環(huán)較多,拔除氣管內(nèi)插管后出現(xiàn)肺水腫,再次氣管內(nèi)插管;4例患者因出血需二次開胸止血,其他患者術后均順利康復出院。體外循環(huán)時間(84.0±22.0) min,主動脈阻斷時間(42.0±12.0) min。術后即刻右心室流出道壓力階差(18.0±4.5) mm Hg,術后最后一次隨訪的經(jīng)胸超聲心動圖檢查提示,右心室流出道壓力階差為(19.2±5.4) mm Hg,兩者差異無統(tǒng)計學意義(P>0.05)。術后即刻肺動脈瓣反流程度:輕微反流32例(1+),輕度反流28例(2+);術后最后一次隨訪的經(jīng)胸超聲心動圖檢查提示,反流程度:輕微28例(1+),輕度27例(2+),中度5例(3+);兩者反流程度差異無統(tǒng)計學意義(P>0.05)。牛頸靜脈帶瓣補片及瓣葉組織均未見鈣化,瓣葉活動良好。無牛頸靜脈血栓形成及感染性心內(nèi)膜炎發(fā)生。?結論?對法洛四聯(lián)癥和右心室雙出口合并肺動脈狹窄的患者,牛頸靜脈帶瓣補片是右心室流出道跨瓣環(huán)重建的一種良好材料,無相關的嚴重并發(fā)癥發(fā)生,近期右心室流出道壓力階差沒有明顯增加,肺動脈瓣抗反流性能良好;遠期效果還需進一步隨訪觀察。
