目的　探討二尖瓣置換術(shù)（MVR）后左心室后壁早期破裂的原因、治療及預(yù)防措施?！》椒ā』仡櫺苑治?981年5月至2010年11月期間沈陽軍區(qū)總醫(yī)院施行二尖瓣置換術(shù)2 638例中發(fā)生左心室后壁早期破裂7例的臨床資料，其中男2例，女 5例；年齡28～71歲。術(shù)前心功能分級（NYHA）Ⅱ級1例，Ⅲ級6例。心臟彩色超聲心動圖提示左心室射血分?jǐn)?shù)（LVEF） 49%～ 60%，左心室舒張期末容積（LVEDV） 29～42 ml。均在中度低溫、全身麻醉、體外循環(huán)下行人工心瓣膜置換術(shù)?！〗Y(jié)果　7例左心室后壁早期破裂均經(jīng)手術(shù)修補，其中Ⅰ型2例，Ⅱ型4例，Ⅲ型1例。死亡4例，均死于失血性休克或低心排血量綜合征。手術(shù)搶救成功3例，術(shù)后7～15 d順利出院。3例生存患者隨訪4.5～18.0年，一般狀況良好，生活質(zhì)量滿意，無晚期假性動脈瘤形成。　結(jié)論　左心室后壁早期破裂修補困難，死亡率較高，應(yīng)以預(yù)防為主；術(shù)中一旦發(fā)生破裂，應(yīng)該盡快恢復(fù)體外循環(huán)，根據(jù)實際情況在心腔內(nèi)、外聯(lián)合修補較為確實、可靠，有可能挽救患者的生命。
