目的?探討合并感染性心內膜炎的肥厚梗阻性心肌?。╤ypertrophic obstructive cardiomyopathy,HOCM)患者的手術方式及療效。?方法?回顧性分析2006年9月至2012年2月阜外心血管病醫(yī)院采用擴大Morrow手術+心內贅生物清除術+受累心瓣膜置換術治療7例合并感染性心內膜炎HOCM患者的臨床資料,其中男5例,女2例;年齡21~55 (39.80±13.60)歲。術后觀察手術療效,比較手術前后的左心室流出道(LVOT)峰值壓差、左心房內徑、左心室射血分數(shù)和心功能。?結果?手術成功率100%,圍術期無死亡。細菌性贅生物位于二尖瓣瓣葉7例、主動脈瓣瓣葉4例、室間隔1例;均為多發(fā)性,贅生物直徑2~19 mm。血培養(yǎng)金黃色葡萄球菌生長3例、松鼠葡萄球菌生長1例。術后發(fā)生Ⅰ°房室傳導阻滯2例、完全性左束支傳導阻滯1例、左前分支傳導阻滯2例,未予特殊處理。術后LVOT峰值壓差、左心房內徑均低于或小于術前;心功能較術前明顯改善。7例患者均獲得隨訪,隨訪時間1~49 (13.00±17.19)個月,所有患者臨床癥狀消失或明顯減輕,生活質量明顯改善,心功能分級(NYHA)Ⅰ~Ⅱ級,無再次手術或死亡。?結論?擴大Morrow手術+受累心瓣膜置換術治療合并感染性心內膜炎HOCM患者,其早中期療效滿意。
