【摘 要】 目的 探討晚期扁桃體癌切除術(shù)后缺損修復(fù)與功能重建的組織瓣供區(qū)選擇。 方法 2000年10月－2010年2月收治13例晚期扁桃體癌患者。男11例，女2例；年齡39～67歲，平均53.6歲。高分化鱗狀細(xì)胞癌5例，中分化鱗狀細(xì)胞癌8例。按國(guó)際抗癌聯(lián)盟（UICC）1997年口咽癌的分期標(biāo)準(zhǔn)：T1N1M0 1例，T2N1M0 2例，T2N2M0 2例，T3N1M0 3例，T3N2M0 2例，T4N1M0 2例，T4N2M0 1例。病程1～8個(gè)月，平均4.3個(gè)月。腫瘤侵及鼻咽側(cè)壁1例、下咽側(cè)壁3例、會(huì)厭1 例、軟腭4例、舌根部3例。腫瘤浸潤(rùn)范圍2 cm × 2 cm～12 cm × 6 cm?；颊呔扇∈中g(shù)加術(shù)后補(bǔ)充放療的綜合治療措施。經(jīng)頸-頜-扁桃體聯(lián)合入路切除腫瘤后，5例采用帶蒂胸大肌皮瓣、5例采用游離前臂皮瓣、3例采用游離股前外側(cè)皮瓣一期修復(fù)。 結(jié)果 術(shù)后病理報(bào)告示頸淋巴結(jié)轉(zhuǎn)移10例；補(bǔ)充放療后8個(gè)月～2年局部復(fù)發(fā)2例，頸部淋巴結(jié)轉(zhuǎn)移3例。術(shù)后1例帶蒂胸大肌皮瓣修復(fù)術(shù)后第5天出現(xiàn)頸部感染，1例游離前臂皮瓣修復(fù)術(shù)后12 h出現(xiàn)血管危象，均經(jīng)對(duì)癥處理后治愈；其余組織瓣均順利成活，切口Ⅰ期愈合。1例股前外側(cè)皮瓣供區(qū)切口裂開(kāi)，二期縫合?；颊呔@隨訪，隨訪時(shí)間1～6年，平均3.6年。5例帶蒂胸大肌皮瓣修復(fù)者，術(shù)后均有不同程度吞咽梗阻感及閉塞性鼻音，言語(yǔ)清晰度較差。8例游離皮瓣修復(fù)者術(shù)后外觀、吞咽及發(fā)聲功能較滿意，供區(qū)無(wú)功能障礙?；颊咝g(shù)后咬關(guān)系均恢復(fù)正常，面部外形和功能基本正常。按直接計(jì)算法，3年生存率為60.0%（6/10），5年生存率為37.5%（3/8）。 結(jié)論 對(duì)晚期扁桃體癌術(shù)后缺損的修復(fù)應(yīng)首選游離前臂皮瓣及股前外側(cè)皮瓣，但對(duì)于組織需要量大和術(shù)前放療失敗的患者，宜選用帶蒂胸大肌皮瓣修復(fù)。
