目的 探討影響Tile C型骨盆損傷髂腰固定術(shù)后療效的相關(guān)因素。 方法2007年3月-2010年3月,收治Tile C型骨盆損傷患者60例。其中男39例,女21例;年齡17~66歲,平均37歲。Tile分型:C1型27例,C2型20例,C3型13例。術(shù)前損傷嚴重程度計分法(ISS)評分12~66分,平均29.4分;傷后至手術(shù)時間2~25 d,平均8.1 d。均采用髂腰固定術(shù)治療,采用非條件logistic回歸模型探討患者年齡、性別、體重指數(shù)、術(shù)前合并傷、手術(shù)時機、功能鍛煉時間、骨折類型、術(shù)后并發(fā)癥、復(fù)位質(zhì)量、骶神經(jīng)損傷程度與術(shù)后療效的關(guān)系。 結(jié)果60例均獲隨訪,隨訪時間12~56個月,平均27.3個月。術(shù)后切口感染、損傷12例,經(jīng)清創(chuàng)、換藥后痊愈;余患者切口均Ⅰ期愈合。其他術(shù)后并發(fā)癥包括遲發(fā)型骶神經(jīng)損害15例,行骶神經(jīng)減壓6例,保守治療9例,病情緩解;髂后上棘處Schanz釘釘尾過度突出10例,局部疼痛不適3 例,取出內(nèi)固定物后緩解;骶髂關(guān)節(jié)植骨不融合1例,給予加壓植骨,植骨融合;橫連桿斷裂5例,對術(shù)后功能無明顯影響;下肢深靜脈血栓形成6例,4例行濾網(wǎng)植入術(shù),2例保守治療?;颊吖桥韫钦塾蠒r間3~6個月,平均3.9個月。末次隨訪時根據(jù)Matta標準評價骨折復(fù)位質(zhì)量,獲優(yōu)31例、良24例、可3例、差2例,優(yōu)良率91.7%。Majeed評分為58~100分,平均86分;獲優(yōu)28例、良12例、可16例、差4例,優(yōu)良率66.7%。logistic回歸分析顯示,年齡、性別、體重指數(shù)、術(shù)后并發(fā)癥與術(shù)后療效無明顯相關(guān);盡早手術(shù)治療(10 d內(nèi))、早期功能鍛煉(7 d以內(nèi))、復(fù)位質(zhì)量越好、骶神經(jīng)損傷程度越輕有利于術(shù)后功能恢復(fù);術(shù)前合并損傷、骨盆損傷越重,術(shù)后療效越差。 結(jié)論手術(shù)時機、術(shù)前合并傷、骨折類型、骨折復(fù)位質(zhì)量、骶神經(jīng)損傷程度、功能鍛煉時間均是影響Tile C型骨盆損傷髂腰固定術(shù)后療效的重要因素。
