目的 總結(jié)封閉式負(fù)壓引流（vacuum sealing drainage，VSD）聯(lián)合游離股前外側(cè)皮瓣移植修復(fù)Pilon骨折術(shù)后軟組織缺損的療效。 方法2008年3月－2011年7月，收治Pilon骨折術(shù)后軟組織缺損患者23例。男15例,女8例；年齡16～63歲，平均32.2歲。Pilon骨折術(shù)后至此次手術(shù)時(shí)間為1～6個(gè)月，平均3.2個(gè)月。創(chuàng)面伴骨、鋼板螺釘外露，創(chuàng)面范圍9 cm × 5 cm～13 cm × 7 cm。VSD治療后，切取大小為10 cm × 6 cm～15 cm × 9 cm的游離股前外側(cè)皮瓣移植修復(fù)創(chuàng)面。供區(qū)直接縫合（6例）或游離植皮（17例）修復(fù)。 結(jié)果術(shù)后3例皮瓣發(fā)生靜脈危象，其余皮瓣均順利成活，創(chuàng)面Ⅰ期愈合。供區(qū)切口均Ⅰ期愈合，皮片除1例出現(xiàn)邊緣部分壞死外，其余均成活。23例均獲隨訪，隨訪時(shí)間6～24個(gè)月，平均11.5個(gè)月。皮瓣質(zhì)地柔軟，外形、色澤好。術(shù)后6個(gè)月踝關(guān)節(jié)功能按照美國(guó)矯形足踝協(xié)會(huì)（AOFAS）評(píng)分系統(tǒng)評(píng)定：獲優(yōu)11例，良8例，可3例，差1例；優(yōu)良率為82.6%。 結(jié)論應(yīng)用VSD聯(lián)合游離股前外側(cè)皮瓣修復(fù)Pilon骨折術(shù)后軟組織缺損，可縮短治療周期，提高手術(shù)成功率，最大限度恢復(fù)肢體功能。
