【摘 要】 目的 探討胸腹部擴(kuò)張后穿支皮瓣跨區(qū)遠(yuǎn)位修復(fù)頜面部及上肢瘢痕切除后創(chuàng)面的方法及療效。 方法 2000 年8 月－ 2011 年2 月，收治25 例頜面部及上肢燒燙傷后瘢痕患者。男14 例，女11 例；年齡7 ～ 36 歲，平均27 歲。頜面部瘢痕15 例，其中雙側(cè)6 例；上肢及手部瘢痕10 例，其中雙側(cè)6 例。瘢痕形成時間6 個月～ 7 年，平均4.5 年。一期
手術(shù)于胸三角及腹部穿支發(fā)出點附近埋置擴(kuò)張器，常規(guī)注水并持續(xù)擴(kuò)張3 個月。二期手術(shù)徹底切除瘢痕后，缺損范圍為
9 cm × 7 cm ～ 17 cm × 8 cm。頜面部瘢痕患者采用大小為17 cm × 7 cm ～ 20 cm × 8 cm 的胸三角穿支皮瓣修復(fù)，上肢及手部瘢痕患者采用大小為10 cm × 9 cm ～ 25 cm × 14 cm 的腹壁下動脈穿支皮瓣修復(fù)。供區(qū)直接拉攏縫合。 結(jié)果 2 例擴(kuò)張過程中未見明顯穿支血管，皮瓣切取后遠(yuǎn)端壞死，經(jīng)換藥及植皮后愈合；其余皮瓣擴(kuò)張過程中均見明顯穿支血管，切
取后皮瓣完全成活。供區(qū)切口均Ⅰ期愈合。10 例獲隨訪，隨訪時間6 個月～ 4 年，平均13 個月。皮瓣色澤、質(zhì)地及彈性佳，
外形滿意。 結(jié)論 應(yīng)用胸腹部擴(kuò)張后穿支皮瓣跨區(qū)遠(yuǎn)位修復(fù)頜面部、上肢及手部瘢痕切除后創(chuàng)面，可獲滿意療效。
