【摘 要】 目的 探討胸鎖乳突肌多頭切斷術(shù)治療成人先天性肌性斜頸的方法及效果。 方法 2009 年3 月－
2011 年2 月，收治19 例成人先天性肌性斜頸患者。男13 例，女6 例；年齡16 ～ 32 歲，平均23.5 歲。X 線片檢查示，12
例伴不同程度頸椎側(cè)彎及楔形改變。10 例伴患側(cè)面部發(fā)育遲緩。4 例既往曾行胸鎖乳突肌單側(cè)頭切斷術(shù)?；颊呔捎眯?br />鎖乳突肌多頭切斷術(shù)，聯(lián)合術(shù)后石膏托及頸托外固定3 ～ 6 個月。 結(jié)果 術(shù)后患者切口均Ⅰ期愈合，無感染及血腫發(fā)生。
16 例患者獲隨訪，隨訪時間5 個月～ 2 年，平均8 個月。患者頭頸部畸形均較術(shù)前明顯改善。術(shù)后2 周療效評定：7 例無
頸椎畸形患者為優(yōu)；12 例伴頸椎畸形患者優(yōu)1 例，良7 例，一般4 例。術(shù)后2 周測量患側(cè)胸鎖乳突肌乳突尖至胸鎖關(guān)節(jié)距
離，無頸椎畸形患者較術(shù)前延長（1.88 ± 0.30）cm，手術(shù)前后差異有統(tǒng)計學(xué)意義（t=6.24，P=0.00），且術(shù)后測量值與正常值比較差異無統(tǒng)計學(xué)意義（t=1.87，P=0.11）；伴頸椎畸形患者術(shù)后較術(shù)前延長（3.38 ± 0.30）cm，差異有統(tǒng)計學(xué)意義（t=11.37，P=0.00），但術(shù)后測量值仍低于正常值（t=12.19，P=0.00）。 結(jié)論 采用胸鎖乳突肌多頭切斷術(shù)治療成人先天性肌性斜頸，能明顯改善癥狀，恢復(fù)頸部活動功能。
