• 遵義醫(yī)學院附屬醫(yī)院燒傷整形外科（貴州遵義，563000）;

【摘 要】 目的  探討幼年豬全陰囊Ⅲ度燒傷后早期植皮修復(fù)陰囊對睪丸生精功能恢復(fù)的影響。 方法 取2月齡雄性貴州小型豬30只，體重10～15 kg；隨機分為3組，每組10只，分別為正常對照組（A組）、自然愈合組（B組）、植皮組（C組）。A組小型豬不作任何處理；B、C組小型豬制備陰囊Ⅲ度燒傷模型后，B組不作處理，待其自行愈合；C組切除燒傷陰囊皮膚，取下腹部全厚皮植皮修復(fù)。術(shù)后觀察B、C組實驗動物一般情況，并于模型制備3個月（實驗動物已成年）及1年每組分別取5只實驗動物雙側(cè)睪丸標本，HE染色觀察不同方式修復(fù)陰囊對睪丸生精細胞的形態(tài)學影響，免疫組織化學染色檢測Survivin蛋白表達。 結(jié)果  實驗動物均存活至實驗完成，創(chuàng)面均順利愈合。組織學觀察示A組各級生精細胞形態(tài)正常，管腔中可見成熟精子；B組生精上皮明顯變薄，僅1～2層生精細胞；C組生精細胞較B組稍多，部分可見精子細胞；術(shù)后1年B、C組生精細胞較3個月時增加。免疫組織化學染色示術(shù)后3個月及1年B、C組Survivin蛋白表達少于A組，B組少于C組，差異均有統(tǒng)計學意義（P  lt; 0.05）；組內(nèi)兩時間點比較，差異均無統(tǒng)計學意義（P  gt; 0.05）。 結(jié)論  植皮修復(fù)全陰囊Ⅲ度燒傷創(chuàng)面對豬睪丸生精功能有抑制作用，可導(dǎo)致生精細胞減少，Survivin蛋白表達降低，但生精小管內(nèi)仍有部分精子細胞殘存。
