目的 比較尺神經(jīng)皮下前置及肌下前置兩種方法治療肘管綜合征的療效，為臨床選擇恰當(dāng)?shù)闹委熜g(shù)式提供依據(jù)。 方法 2006 年6 月－ 2008 年10 月收治39 例肘管綜合征患者，其中20 例采用尺神經(jīng)皮下前置（皮下前置組），19 例采用尺神經(jīng)肌下前置（肌下前置組）。兩組患者性別、年齡、病程及臨床分型等一般資料比較，差異均無(wú)統(tǒng)計(jì)學(xué)意義（P  gt; 0.05），具有可比性。 結(jié)果 兩組患者術(shù)后切口均Ⅰ期愈合。術(shù)后肌下前置組17 例（89.5%）發(fā)生尺神經(jīng)卡壓癥狀緩解后突然加重，1 例（5.3%）肘部瘢痕增生；皮下前置組10 例（50.0%）出現(xiàn)觸碰肘前內(nèi)側(cè)皮膚時(shí)手部尺側(cè)麻木；兩組并發(fā)癥發(fā)生情況比較，差異有統(tǒng)計(jì)學(xué)意義（χ2=9.632，P=0.002）?；颊呔@隨訪，隨訪時(shí)間24 ～ 36 個(gè)月，平均28 個(gè)月。末次隨訪時(shí)，兩組手部握力，拇、環(huán)指及拇、小指捏力，以及小指末節(jié)兩點(diǎn)辨別覺比較，差異均無(wú)統(tǒng)計(jì)學(xué)意義（P  gt; 0.05）；但均較術(shù)前顯著改善，差異均有統(tǒng)計(jì)學(xué)意義（P  lt; 0.05）。按中華醫(yī)學(xué)會(huì)手外科學(xué)會(huì)上肢部分功能評(píng)定試用標(biāo)準(zhǔn)評(píng)定：皮下前置組獲優(yōu)5 例，良12 例，可1 例，差2 例；肌下前置組獲優(yōu)6 例，良10 例，可2 例，差1 例；兩組比較差異無(wú)統(tǒng)計(jì)學(xué)意義（u=0.346，P=0.734）。參照患者源性功能調(diào)查表上肢功能評(píng)定表評(píng)定：皮下前置組為（22 ± 7）分，肌下前置組為（19 ± 6）分，差異無(wú)統(tǒng)計(jì)學(xué)意義（t=1.434，P=0.161）。 結(jié)論 尺神經(jīng)皮下前置及肌下前置兩種方法治療肘管綜合征均可達(dá)到良好療效，其中尺神經(jīng)肌下前置法并發(fā)癥較少。
