目的 總結(jié)全手皮膚脫套傷的傷情特點，探討全手皮膚脫套傷的分型標(biāo)準(zhǔn)及治療方法。 方法 1999年12 月－ 2010 年5 月，收治41 例全手皮膚脫套傷。男28 例，女13 例；年齡18 ～ 58 歲，平均35 歲。致傷原因：碾壓傷28 例，擠壓傷13 例。受傷至手術(shù)時間1 ～ 10 h，平均3 h。根據(jù)自定全手皮膚脫套傷分型標(biāo)準(zhǔn)：Ⅰ型11 例，Ⅱ型5 例，Ⅲ型4 例，Ⅳ型8 例，Ⅴ型13 例。Ⅰ型采用吻合血管回植術(shù)；Ⅱ型采用帶足背皮瓣的甲瓣、第2 趾甲瓣再造術(shù)；Ⅲ型采用雙足帶足背皮瓣的第2 趾甲瓣再造術(shù)；Ⅳ型采用吻合血管回植術(shù)；Ⅴ型采用帶足背皮瓣的甲瓣再造（8 例）或腹部皮瓣修復(fù)術(shù)（5 例）。足背皮瓣切取范圍為9 cm × 6 cm ～ 17 cm × 11 cm，足背供區(qū)游離植皮修復(fù)。 結(jié)果 術(shù)后Ⅰ型6 例發(fā)生部分手指壞死，Ⅳ型6 例發(fā)生部分手指及手掌皮膚壞死；其余患者皮瓣、再造指及回植皮膚均成活。足背供區(qū)及腹部供區(qū)均順利愈合。40 例患者獲隨訪，隨訪時間6 個月～ 7 年，平均14 個月。采用吻合血管回植治療者，手部皮膚顏色、質(zhì)地接近正常，功能恢復(fù)佳，感覺恢復(fù)至S2 ～ S4；采用甲瓣及趾甲瓣再造手指者，手功能基本恢復(fù)，再造指感覺恢復(fù)至S2 ～ S3；采用腹部皮瓣者，手功能恢復(fù)欠佳，手部感覺恢復(fù)至S1 ～ S2。 結(jié)論 采用自定標(biāo)準(zhǔn)對全手皮膚脫套傷程度進(jìn)行分型，并指導(dǎo)臨床治療方案的選擇，可獲得較好臨床療效。
