目的 報(bào)道1 例應(yīng)用游離延展上臂外側(cè)皮瓣修復(fù)洞穿性頰部缺損的臨床效果。 方法 2011 年2 月收治1 例53 歲右頰中分化鱗狀細(xì)胞癌男性患者，腫瘤根治及右側(cè)頸淋巴清掃術(shù)后遺留洞穿性頰部缺損約5 cm × 5 cm。設(shè)計(jì)以左側(cè)后橈側(cè)副動(dòng)脈及其伴行靜脈為蒂的上臂外側(cè)皮瓣，并擴(kuò)展至前臂上部，皮瓣切取面積約9 cm × 5 cm，延長(zhǎng)后蒂長(zhǎng)約10 cm，皮瓣折疊后修復(fù)軟組織缺損。 結(jié)果 患者術(shù)后創(chuàng)面Ⅰ期愈合，皮瓣成活。術(shù)后病理提示右頸深上淋巴結(jié)轉(zhuǎn)移性鱗癌，因此術(shù)后1 個(gè)月接受劑量約60 Gy 放射治療。患者獲隨訪6 個(gè)月，皮瓣色澤、質(zhì)地良好，口輪匝肌功能正常，患側(cè)口角無(wú)歪斜，張口度約3 cm。左側(cè)上臂無(wú)功能障礙，但創(chuàng)面周圍3 cm 區(qū)域仍有麻木感。 結(jié)論 游離延展上臂外側(cè)皮瓣血供可靠，操作簡(jiǎn)便、安全，是一種修復(fù)洞穿性頰部軟組織缺損的較好選擇。
