目的 探討改良法經(jīng)病椎椎弓根植釘、復(fù)位、植骨及后路釘棒系統(tǒng)治療胸腰椎爆裂性骨折的方法和臨床療效。 方法 2008 年3 月－ 2010 年9 月，對(duì)21 例胸腰椎爆裂性骨折患者行經(jīng)病椎椎弓根植釘、復(fù)位、植骨及后路釘棒系統(tǒng)內(nèi)固定術(shù)。其中男15 例，女6 例；年齡20 ～ 61 歲，平均38.4 歲。累及節(jié)段：T12 5 例，L1 7 例，L2 5 例，T12 ～ L1 4例。Frankel 神經(jīng)功能分級(jí)：A 級(jí)2 例，B 級(jí)4 例，C 級(jí)6 例，D 級(jí)5 例，E 級(jí)4 例。按Denis 分型均為爆裂性骨折，其中A型7 例，B 型11 例，C 型3 例。術(shù)前、術(shù)后及隨訪時(shí)行X 線片檢查測(cè)量傷椎椎體高度、后凸Cobb 角，評(píng)價(jià)神經(jīng)功能改善情況。 結(jié)果 患者術(shù)后切口均Ⅰ期愈合。21 例均獲隨訪，隨訪時(shí)間12 ～ 30 個(gè)月，平均26 個(gè)月。無(wú)內(nèi)固定物松動(dòng)及斷裂發(fā)生。X 線片示骨折均達(dá)骨性愈合，愈合時(shí)間12 ～ 23 個(gè)月，平均16 個(gè)月。術(shù)后1 周及1 年時(shí)后凸Cobb 角分別為（3.4 ± 2.4）° 和（5.2 ± 3.2）°，兩時(shí)間點(diǎn)間變化不明顯，差異無(wú)統(tǒng)計(jì)學(xué)意義（P  gt; 0.05），但均較術(shù)前（22.1 ± 1.2）° 顯著降低，差異有統(tǒng)計(jì)學(xué)意義（P  lt; 0.05）。術(shù)后1 周傷椎前緣高度恢復(fù)至（36.0 ± 2.0） mm，傷椎前緣高度與正常相鄰上下位椎體高度比值為95.3% ± 1.3%，術(shù)后1 年分別為（35.0 ± 2.4） mm 和94.4% ± 2.5%，兩時(shí)間點(diǎn)間高度丟失不明顯（P  gt; 0.05），但均較術(shù)前（14.6 ± 2.1） mm 和40.2% ± 1.5% 顯著提高（P  lt; 0.05）。術(shù)后1 年患者Frankel 神經(jīng)功能分級(jí)較術(shù)前有不同程度恢復(fù)，與術(shù)前比較差異有統(tǒng)計(jì)學(xué)意義（χ2=11.140，P=0.025）。 結(jié)論 改良法經(jīng)病椎椎弓根植釘、復(fù)位、植骨以及后路釘棒系統(tǒng)治療胸腰椎爆裂性骨折可重建脊柱前中柱穩(wěn)定性，防止后期矯正角度及椎體高度的再丟失。
