目的 探討開胸體外循環(huán)直視手術(shù)、解剖外旁路聯(lián)合動(dòng)脈腔內(nèi)修復(fù)(endovascular aneurysm repair,EVAR)雜交手術(shù)以及完全EVAR 手術(shù)治療主動(dòng)脈弓降部病變的方法及療效。 方法 2006 年10 月- 2011 年9 月,收治48 例主動(dòng)脈弓降部病變患者。男31 例,女17 例;年齡28 ~ 81 歲,平均52.4 歲。病程1 ~ 90 d,平均10.2 d。累及弓部分支的B 型主動(dòng)脈夾層30 例,主動(dòng)脈弓降部真性動(dòng)脈瘤11 例,主動(dòng)脈弓降部假性動(dòng)脈瘤3 例,主動(dòng)脈弓穿透性潰瘍伴壁間血腫3 例,主動(dòng)脈食管瘺1 例。15 例行開胸體外循環(huán)直視手術(shù),12 例行解剖外旁路聯(lián)合EVAR 雜交手術(shù),21 例行完全EVAR 手術(shù)。 結(jié)果 開胸體外循環(huán)直視手術(shù)患者中術(shù)后發(fā)生出血1 例,昏迷1 例,短暫精神癥狀3 例,肺炎4 例,急性腎功能不全2 例,多器官功能衰竭2 例;最終3 例死亡。解剖外旁路聯(lián)合EVAR 雜交手術(shù)患者術(shù)后1 例出現(xiàn)右頂枕葉大面積梗死伴肺炎、腎功能衰竭。完全EVAR 術(shù)后無(wú)并發(fā)癥發(fā)生。術(shù)后41 例獲隨訪,隨訪時(shí)間2 ~ 60 個(gè)月,平均28.6個(gè)月。3 例患者出現(xiàn)左鎖骨下動(dòng)脈竊血綜合征表現(xiàn),因癥狀輕微,未予特殊處理,均自行緩解。其余患者均恢復(fù)正常生活。 結(jié) 論 對(duì)于主動(dòng)脈弓降部病變,采用開胸體外循環(huán)直視手術(shù)創(chuàng)傷大、風(fēng)險(xiǎn)高,將逐步被EVAR 替代,解剖外旁路聯(lián)合EVAR 雜交手術(shù)是治療此病變的重要方法,完全EVAR 手術(shù)是其發(fā)展方向。
