目的比較防旋型股骨近端髓內(nèi)釘(proximal femoral nail antirotation, PFNA)與重建釘微創(chuàng)治療青壯年股骨干骨折合并同側(cè)髖部“囊外型”骨折的療效。 方法回顧性分析2000年1月-2010年8月采用PFNA和重建釘微創(chuàng)治療的69例青壯年股骨干骨折合并同側(cè)髖部“囊外型”骨折患者臨床資料,其中重建釘組44例,PFNA組25例。兩組患者性別、年齡、體重、致傷原因、骨折類型及受傷至手術(shù)時(shí)間比較,差異均無統(tǒng)計(jì)學(xué)意義(P  gt; 0.05),具有可比性。對(duì)兩組手術(shù)時(shí)間、術(shù)中失血量、骨折愈合時(shí)間、并發(fā)癥及髖、膝關(guān)節(jié)功能等進(jìn)行比較。 結(jié)果PFNA組手術(shù)時(shí)間及術(shù)中失血量均明顯少于重建釘組,差異有統(tǒng)計(jì)學(xué)意義(P  lt; 0.05)。69例均獲1年以上隨訪,PFNA組隨訪時(shí)間12~38個(gè)月,平均20個(gè)月;重建釘組隨訪時(shí)間12~48個(gè)月,平均22個(gè)月。兩組均無傷口感染、深靜脈血栓形成、肺栓塞、內(nèi)固定斷裂、股骨頭缺血性壞死及肢體嚴(yán)重旋轉(zhuǎn)與短縮畸形等并發(fā)癥發(fā)生。術(shù)中PFNA組植釘困難1例,重建釘組近端鎖釘技術(shù)困難7例;術(shù)中PFNA組醫(yī)源性股骨干骨折3例,重建釘組6例;術(shù)后PFNA組股骨干骨折延遲愈合1例,重建釘組2例;PFNA組和重建釘組并發(fā)癥發(fā)生率分別為20%(5/25)和34%(15/44),比較差異無統(tǒng)計(jì)學(xué)意義(χ2=1.538,P=0.215)。兩組各部位骨折愈合時(shí)間比較差異均無統(tǒng)計(jì)學(xué)意義(P  gt; 0.05)。末次隨訪時(shí),兩組髖關(guān)節(jié)Harris評(píng)分和膝關(guān)節(jié)Evanich評(píng)分比較差異均無統(tǒng)計(jì)學(xué)意義(P  gt; 0.05)。 結(jié)論P(yáng)FNA與重建釘固定股骨干骨折合并同側(cè)髖部“囊外型”骨折的臨床療效均滿意,但PFNA具有操作簡便等優(yōu)點(diǎn)。
