目的探討手術(shù)治療駕駛摩托車致膝關(guān)節(jié)前內(nèi)側(cè)旋轉(zhuǎn)不穩(wěn)定（antero-medial rotatory instability，AMRI）的效果。 方法2007年6月－2009年12月，收治32例駕駛摩托車致膝關(guān)節(jié)AMRI患者。男28例，女4例；年齡20～50歲，平均35.5歲。受傷至手術(shù)時(shí)間5～10 d，平均7 d。前交叉韌帶（anterior cruciate ligament，ACL）均于脛骨髁間嵴附著點(diǎn)處撕脫；內(nèi)側(cè)副韌帶（medial collateral ligament，MCL）損傷部位：中央部位19例，股骨內(nèi)側(cè)髁部10例，脛骨內(nèi)側(cè)髁部3例。均為閉合損傷。采用鋼絲固定脛骨髁間嵴撕脫骨折塊，同時(shí)修復(fù)MCL治療。 結(jié)果術(shù)后1例發(fā)生切口紅腫伴少量滲液，其余患者切口均Ⅰ期愈合。5例發(fā)生膝關(guān)節(jié)創(chuàng)傷性關(guān)節(jié)炎，經(jīng)相應(yīng)處理后好轉(zhuǎn)。術(shù)后患者均獲隨訪，隨訪時(shí)間16～22個(gè)月，平均18.5個(gè)月。X線片檢查示脛骨髁間嵴骨折均于術(shù)后5～8周愈合，平均6周。末次隨訪時(shí)，患者膝關(guān)節(jié)伸膝均達(dá)0°；屈膝110～170°，平均155°。采用國際膝關(guān)節(jié)文獻(xiàn)委員會（IKDC）分級標(biāo)準(zhǔn)評價(jià)膝關(guān)節(jié)功能，獲A級24例，B級6例，C級1例，D級1例。Lysholm膝關(guān)節(jié)評分為（85.93 ± 3.76）分，明顯高于術(shù)前的（37.54 ± 3.43）分，差異有統(tǒng)計(jì)學(xué)意義（t=53.785，P=0.000）。 結(jié)論駕駛摩托車致膝關(guān)節(jié)AMRI，采用鋼絲固定脛骨髁間嵴撕脫骨折塊，同時(shí)修復(fù)MCL治療，結(jié)合術(shù)后早期功能鍛煉，近期療效滿意，遠(yuǎn)期療效尚需隨訪觀察。
